The Dietitian possesses clinical expertise in care related to diabetes care and management, including "self-management." This role provides education on nutritional status, nutrition care planning, physical activity, disease process, diabetes medication, problem-solving, coping, and risk reduction. The Dietitian also informs patients about the important factors associated with the disease and the preventive measures they need to consider while treating it. Supports patients in improving and maintaining their overall health. Provides patient, staff, and community education.

As part of this role, you will:

Promote the mission, vision, and values of the organization
• Provide outpatient diabetes self-management training and education to type 1, type 2, and gestational diabetes patients
• Plan, develop, and implement individual and group diabetes education for adults, their families, and caregivers
• Instruct patients in the Standards of Care
• Provide education related to diabetes and the disease process; medication, diet, and nutritional monitoring, foot care prevention, and the techniques of self-monitoring and injection
• Advise patients as to the potential for long-term complications
• Provide ongoing assessment and recommendations to the treatment team and patient/family for adjustments in the plan of care as required
• Participates with the medical team, assuring diabetes clinical care and management are consistent with ADA standards of care and guidelines
• Document assessments, plans, and goals in the Electronic Medical Record
• Identifies opportunities to improve patient/customer experience. Participates in projects/programs designed to enhance service delivery and patient/customer satisfaction
• Evaluates the diabetes and health education program on a continuing basis; assesses teaching methods and educational material for both staff and the families of patients
• Perform assessments and data collections, establish outcome criteria; obtain reviews, and analyze information in relation to HEDIS measure compliance
• Utilizes a team approach to provide services, and collaborates and communicates with the team members when needed

What you need to bring to this role:

  • Unrestricted and Current Florida License 
  • Excellent active listening and interpersonal skills
  • Must be flexible, resourceful, and able to troubleshoot
  • Must be able to handle multiple tasks simultaneously and set priorities
  • Bilingual (Spanish/English) helpful but not required
